ChenMed is currently seeking a Primary Care Physician (PCP) to join their team. This role is more than traditional patient care; it is a leadership position demanding accountability for patient outcomes, coaching patients to achieve health goals, and simplifying complex medical processes to create actionable care plans. The PCP will manage a panel of up to 450 patients, integrating clinical care with cost-effective strategies to improve both individual health outcomes and broader organizational metrics. Working closely with multidisciplinary care teams, the PCP will lead and support efforts that enhance patient experience, close clinical care gaps, and reduce unnecessary hospital admissions.

Job Requirements
- Valid medical license
- board certification in family medicine, internal medicine, or geriatrics
- minimum of 3 years clinical experience in primary care
- ability to manage a panel of up to 450 patients
- excellent clinical judgment and independent decision-making ability
- proficiency in English communication
- willingness to travel locally and nationally for training and recruitment
- proficiency with Microsoft Office and electronic health records
- dedication to patient-centered care and quality improvement
- availability and accessibility to patients
- commitment to physician wellness and self-care
Job Qualifications
- Medical degree from an accredited institution
- board certification in primary care
- valid medical license
- experience in managing acute and chronic conditions in older adults
- strong leadership and team collaboration skills
- proficiency in electronic medical records and Microsoft Office Suite
- excellent communication and patient coaching abilities
- commitment to evidence-based practice
- ability to influence patients and teams positively
- demonstrated physician leadership and quality improvement engagement
Job Duties
- Provide care independently for patients with acute and chronic illnesses
- take full accountability for patient care and outcomes
- consult specialists appropriately while remaining responsible for patient management
- engage with hospitalists for patients hospitalized
- assess, diagnose, treat, manage, educate, promote health, coordinate care and document for patients
- lead the care team including medical assistants and coordinators
- collaborate with transitional care team and case managers for patients unable to visit the office
- manage center operations and cover for other providers as needed
- participate actively in recruiting patients and providers
- perform other duties as assigned
