The Primary Care Physician (PCP) role at ChenMed is a critical leadership position within this mission-driven company. Each PCP manages a panel of up to 450 patients, taking full accountability for clinical outcomes and cost-effectiveness. The role blends clinical expertise with patient-centered care coaching, focusing on health promotion and behavior change rather than merely treating sickness. Providers serve as health coaches who partner with patients to set and pursue short- and long-term health goals, offering frequent follow-up and support to optimize health outcomes.

PCPs are expected to demonstrate outstanding leadership, both within their care teams and the broader organization culture. They lead teams that include care promoters (medical assistants), care facilitators, and care coordinators to deliver comprehensive, coordinated care. The position involves engagement with hospitalists and transitional care teams for patients unable to attend office visits, ensuring continuity and quality across diverse care settings including hospitals, skilled nursing facilities, and homebound care.

ChenMed expects its PCPs to be active participants in their center's management activities, help recruit patients and providers, and support the organizational culture of love, accountability, and passion. PCPs must work collaboratively with other healthcare professionals and management to comply with regulatory requirements and contribute to risk, quality management, and clinical performance initiatives.

This role requires strong independent judgment, flexibility to work various hours including evenings and weekends, and proficiency in commonly used software tools. PCPs must maintain necessary state medical licenses and board certification and demonstrate ongoing professional development.
