The Primary Care Physician (PCP) role at ChenMed is integral to the organization's mission to provide exceptional care to senior patients. The PCP manages a panel of up to 450 patients, demonstrating accountability for clinical outcomes and cost-effectiveness while acting as a health coach rather than merely a consultant for treating illness. Responsibilities include building trusting relationships, prioritizing and simplifying treatment plans to facilitate behavior change, and actively engaging in achieving key clinical metrics, such as patient admissions per thousand, appointment utilization, patient experience scores, clinical gap closures, and cost management. Physicians at ChenMed lead care teams consisting of medical assistants, care facilitators, and care coordinators and collaborate with hospitalists and transitional care teams to ensure comprehensive patient management across various settings.

This position requires a dynamic physician who takes leadership seriously, contributing to a positive work culture and participating in quality management, compliance, and recruiting efforts. The PCP must be comfortable working independently and collaboratively, remain abreast of evidence-based medicine while considering lifestyle and nutritional factors, and continuously develop leadership and clinical skills. Proficiency in Microsoft Office and other software is expected, along with fluency in English and a readiness to travel locally and nationally as needed. Job Requirements

  • Medical degree from accredited institution
  • Board certification in internal or family medicine
  • Valid medical license to practice
  • Experience in primary care, preferably with older adult patients
  • Proficient with Microsoft Office and related software
  • Fluency in English, verbal and written
  • Ability and willingness to travel locally and nationally
  • Ability to exercise independent judgment
  • Availability and accessibility to patients
  • Commitment to high-quality patient care and compliance
  • Willingness to lead and participate in team activities

Job Duties

  • Provide independent care for patients with acute and chronic illnesses
  • Take accountability for patient care and outcomes
  • Consult specialists as needed while maintaining responsibility
  • Engage with hospitalists for inpatient care coordination
  • Assess, diagnose, treat, manage, educate, and promote health for patients
  • Lead care teams including medical assistants and care coordinators
  • Collaborate with transitional care teams for patients unable to visit the office
  • Participate in management and cover for other providers
  • Supervise and work with Advanced Practice Practitioners
  • Perform additional duties as assigned
