Unlike traditional primary care roles, the PCP functions as more than a medical consultant; they act as a health coach who partners closely with patients to set and achieve meaningful health goals. Each PCP manages a patient panel limited to around 450 individuals, ensuring personalized, accountable, and cost-effective care. PCPs are measured by key performance metrics including patient admissions, appointment utilization, patient experience scores (CGCAHPS), clinical gap closures, and cost management.\n\nPCPs at ChenMed are expected to lead their care teams, which comprise care promoters (medical assistants), care facilitators, and care coordinators. They provide comprehensive care not only to patients visiting the center but also to those in hospitals, skilled nursing facilities, long-term care, or homebound settings by collaborating with transitional care teams and case managers. Leadership within the center is a key responsibility, with PCPs contributing to maintaining a culture of love, accountability, and passion. They also support recruiting efforts for patients and providers, covering for colleagues as needed, and supervising advanced practice practitioners.\n\nChenMed values independent judgment balanced with collaborative healthcare delivery. PCPs engage actively with hospitalists and specialist consultants to ensure continuity and quality of care. They are expected to comply with all company policies, regulatory standards, and quality metrics, participating in risk management programs and clinical meetings dedicated to improving patient outcomes.\n\nThe organization prioritizes competencies such as availability and accessibility to patients, service orientation, evidence-based medicine combined with lifestyle and nutritional insights, and ongoing physician leadership development. PCP wellness and self-care are also emphasized to promote sustainability and maximize the positive impact on patients, staff, and the broader community. Proficiency in Microsoft Office, willingness to travel for training and recruitment, and excellent communication skills are required.\n\nEducationally, candidates must possess an MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Active medical licensure and board eligibility or certification are mandatory, as well as certification in Basic Life Support. The pay range for the role is from $221,141 to $315,915 annually, with final compensation based on experience and other factors. Additional bonuses or commissions may apply. Job Requirements

  • MD or DO degree
  • Ability to obtain or hold current State Medical License
  • Board eligibility or certification in Internal Medicine, Family Medicine, Geriatrics, or similar
  • Current DEA license
  • BLS certification within 90 days
  • Proficiency in English both verbal and written
  • Availability to work evenings, weekends, and holidays
  • Capacity to exercise independent judgment
  • Willingness to travel locally and nationally
  • Commitment to evidence-based and holistic medicine
  • Demonstrated leadership and collaboration skills

Job Qualifications

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • Board certification or eligibility in relevant specialty
  • Ability to obtain and maintain active State Medical License
  • Current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification within first 90 days
  • Experience in providing comprehensive primary care for older adults
  • Proven leadership and team management skills
  • Proficient in Microsoft Office Suite
  • Strong communication and interpersonal skills
  • Willingness to travel locally and nationally as required

Job Duties

  • Provide care independently for patients with acute and chronic illnesses in older adults
  • Take full accountability for patient care and outcomes, consult specialists as needed
  • Engage with hospitalists during patient hospitalizations
  • Assess, diagnose, treat, manage, educate, promote health, coordinate care and document for patients with complex needs
  • Lead care teams including medical assistants, facilitators, and coordinators
  • Collaborate with transitional care teams for non-office bound patients
  • Manage center operations, cover for other providers, and participate in recruiting efforts
  • Supervise and collaborate with advanced practice practitioners
  • Perform other duties as assigned by management
