The Primary Care Physician (PCP) role at ChenMed is both challenging and rewarding, offering the chance to make a meaningful impact on the lives of older adults. PCPs are accountable for the health outcomes of their panel of up to 450 patients, managing acute and chronic illnesses while building trusting relationships that motivate behavioral change. They act as health coaches, setting and following up on short- and long-term health goals with their patients. The PCP is responsible for simplifying care plans to make them more actionable for patients and leading their care teams to fulfill performance objectives. Regular performance metrics such as admission rates, appointment utilization, patient experience scores, and cost control measures are used to track success, with PCPs expected to work collaboratively with center leadership and care teams to meet these goals.

PCPs at ChenMed play a central leadership role within their centers, fostering a culture of accountability, passion, and love alongside management. They must be comfortable working both independently and as part of an interdisciplinary healthcare team, engaging hospitalists, transitional care teams, case managers, and other specialists to ensure comprehensive care coordination. Responsibilities include assessment, diagnosis, treatment, education, and follow-up for complex health needs, both in the office and across care settings such as hospitals, SNFs, LTCs, and homebound patient visits. PCPs also support center operations by covering for absent colleagues and contributing to patient recruitment and provider hiring efforts.

The role requires proficiency in contemporary electronic medical record systems and Microsoft Office applications, with a strong emphasis on evidence-based medicine complemented by lifestyle and nutrition interventions. Excellent physician leadership skills are essential, as PCPs influence their patients, care teams, and organizational culture to achieve high-quality outcomes. They must practice self-care to sustain their well-being and effectiveness in this demanding role. PCPs should be fluent in English; additional language skills may be required in some locations. The position includes local and occasional regional or national travel for training and recruitment purposes.

Job Requirements

  • Must have or be able to obtain a state medical license for practicing state(s)
  • MD or DO degree from an accredited medical school
  • Board eligibility in Internal Medicine, Family Medicine, Geriatrics or similar specialty
  • Current DEA licensure for controlled substances
  • BLS certification from American Heart Association or American Red Cross within 90 days
  • Demonstrated physician leadership skills and commitment to evidence-based medicine
  • Excellent communication and interpersonal skills for patient and team interaction
  • Ability to maintain quality standards and regulatory compliance
  • Must be available and accessible to patients including walk-in visits and timely responses
  • Ability to travel as required for training and recruitment purposes

Job Qualifications

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
  • Board eligibility required, board certification preferred
  • Current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification within first 90 days of employment
  • Proficiency in Microsoft Office Suite and healthcare software
  • Fluency in English, verbal and written
  • Ability to function independently and collaboratively
  • Willingness and ability to travel locally, regionally, and nationally as required

Job Duties

  • Independently provide care for patients with acute and chronic illnesses encountered in older adults
  • Take full accountability for patient care and outcomes and seek specialist consultation when necessary
  • Engage with hospitalists when patients are hospitalized regardless of hospitalist affiliation
  • Assess, diagnose, treat, manage, educate, promote health, coordinate care and document for patients with acute and complex chronic health needs
  • Lead a care team including care promoters, facilitators, and coordinators for office-based patients
  • Collaborate with transitional care teams, case managers, and acute care physicians for patients unable to visit the office
  • Participate in center management, cover for absent providers, and assist in patient and provider recruitment
  • Perform other duties as assigned by management
