The Primary Care Physician at ChenMed holds a crucial role in managing a panel of up to 450 patients, focusing on delivering both clinical excellence and cost-effective care. Unlike traditional clinician roles, the PCP acts as a health coach who builds trusting relationships, encouraging patients to pursue and maintain short- and long-term health goals. This approach actively supports lifestyle modifications and preventive care, simplifying complex medical needs into actionable steps. PCPs are deeply involved in their patients’ continuum of care, collaborating with hospitalists, specialists, and care teams to ensure seamless transitions and high-quality outcomes.

In this role, PCPs are expected to demonstrate unwavering accountability for patient outcomes, including monitoring clinical metrics such as patient admissions, patient experience scores (CGCAHPS), clinical gap closures, and medical cost management. The role demands a dedication to evidence-based medicine integrated with understanding of non-traditional factors such as nutrition and lifestyle that influence health. PCPs contribute actively to center management, recruiting efforts, and enhancing team performance. They lead multidisciplinary care teams consisting of care promoters, facilitators, and coordinators to provide comprehensive care both in clinical settings and for patients receiving care in hospitals, skilled nursing facilities, long-term care, or homebound situations.

ChenMed values PCPs who exemplify strong physician leadership, consistently seeking to improve their leadership skills to benefit patients, teams, and the company culture. PCPs function both independently and collaboratively within established practice agreements and maintain accountability for the oversight of advanced practice providers. This role offers a competitive salary range from $221,141 to $315,915 annually, with additional eligibility for bonuses or commissions.
