Job Requirements
- Active California NP and RN licenses
- active furnishing license
- valid NPI number
- DEA certification
- BLS certification
- minimum 2 years of experience in geriatrics or related facility-based care
- ability to travel within Stockton, Modesto, and Amador
- proficiency in EMR use
- valid California driver’s license
- reliable transportation

- proficiency with EMR systems
- strong communication and interpersonal skills
Job Duties
- Conduct routine and urgent health assessments
- manage chronic diseases
- perform post-discharge follow-ups including medication reconciliation and ordering labs/diagnostics
- coordinate care with facility staff, specialists, and families/POAs
- complete same-day documentation and ensure CMS/regulatory compliance
- maintain accurate medical records in the EMR system
- participate in scheduled patient visits across Stockton, Modesto, and Amador
