We are currently seeking highly skilled Primary Care Nurse Practitioners (NPs) to join our growing team. In this role, you will provide on-site primary care services to residents in Assisted Living and Residential Care Facilities for the Elderly (RCFE) across the Stockton, Modesto, and Amador regions. This full-time position combines direct patient care with remote administrative work to ensure a balanced workload and work-life integration. You will conduct routine and urgent health assessments, manage chronic conditions, and perform post-discharge follow-ups including medication reconciliation and ordering of labs and diagnostics. Coordination with facility staff, specialists, and families or Power of Attorneys (POAs) is a key aspect of this role, ensuring continuity of care and optimizing patient outcomes. You will also maintain compliance with Centers for Medicare & Medicaid Services (CMS) and other regulatory standards by completing same-day documentation using advanced digital tools.
The schedule is designed to maximize efficiency and minimize travel. You will spend three days a week delivering bedside care across strategically clustered routes within the designated service areas, followed by two remote days for completing documentation, care coordination, and other administrative tasks from home. This hybrid model offers both in-person patient interaction and flexible remote working opportunities.
The compensation package for this role is competitive, with an annual salary range between $140,000 and $175,000 dependent upon experience, panel size, and schedule. Additional earning potentials are available based on coverage and panel demands. - Active California NP and RN licenses
- active furnishing license
- valid NPI number
- DEA certification
- BLS certification
- minimum 2 years of experience in geriatrics or related facility-based care
- ability to travel within Stockton, Modesto, and Amador
- proficiency in EMR use
- valid California driver’s license
- reliable transportation
