We are currently seeking dedicated Primary Care Nurse Practitioners (NPs) to join our dynamic team and provide on-site primary care services to assisted living residents across Stockton, Modesto, and Amador. This role is ideal for NPs who are passionate about geriatrics and want to make a meaningful difference in the lives of seniors by offering routine and urgent assessments, chronic disease management, and post-discharge follow-ups. The position offers a full-time weekday schedule that balances in-person care with remote work, enabling providers to maintain an effective work-life balance while serving an important community need.

The Primary Care NP will be responsible for delivering high-quality clinical care by performing thorough assessments, managing chronic illnesses, coordinating with facility staff, specialists, families, and power of attorneys (POAs), and ensuring timely post-discharge care such as medication reconciliation and lab diagnostics. Documentation is a critical aspect of this role, and providers are expected to complete same-day charting in compliance with CMS and other regulatory standards. To support providers, AllCare.ai offers modern technological tools and 24/7 concierge assistance for scheduling, coordination, lab orders, and pharmacy needs, thereby minimizing administrative obstacles and allowing providers to focus more on patient care.

This opportunity features strategically clustered patient care routes designed to minimize travel time across multiple facilities, promoting efficient workflow and maximizing direct patient interaction. On-site care is delivered three days per week, with the remaining two days reserved for remote administrative and documentation tasks. This hybrid model caters to those seeking a rewarding clinical role with flexibility and modern support infrastructure.

Candidates should have active California NP and RN licenses, an active furnishing license, and credentials including NPI, DEA, and BLS certification. Experience in geriatrics or facility-based practice is preferred, alongside proficiency with electronic medical records (EMR). Compensation ranges from $150,000 to $175,000 annually, commensurate with experience, schedule, and patient volume. Additional earning opportunities exist based on coverage and panel needs. This is a W-2 position with full benefits, including paid time off and mileage reimbursement according to company policy. Final compensation and terms will be detailed in the employment agreement.
