The Primary Care Nurse Practitioner role at AllCare.ai is a dynamic blend of hands-on, on-site patient care and remote administrative functions, designed to optimize both caregiver effectiveness and patient outcomes. This position entails visiting strategically clustered Assisted Living and RCFE facilities in the aforementioned regions to perform routine and urgent assessments, manage chronic diseases, and execute post-discharge follow-ups. Additionally, nurse practitioners are responsible for medication reconciliation, ordering and reviewing labs and diagnostics, and coordinating care with facility staff and specialists. Utilizing modern tools that automate charting and ensure compliance with CMS and other regulatory requirements, practitioners can complete same-day documentation with ease, reducing administrative workload.

This full-time, weekday position offers a structured schedule with three days dedicated to delivering direct patient care on-site, and two days reserved for remote administrative and documentation tasks. The approach is designed to maximize patient interaction while minimizing travel and administrative strain. Financially, the role is competitive, with an annual compensation range between $150,000 and $175,000, which varies based on experience, schedule, and patient volume. The compensation package also includes potential additional earnings based on coverage and patient panel needs.
