The position of Post Acute Coordinator within the Lafayette Franciscan Healthy Living Clinic plays an essential role in the continuum of patient care, specifically facilitating admissions to post-acute service lines which include Home Health, Hospice, and the PACE (Program of All-Inclusive Care for the Elderly) program. This role is responsible for ensuring a smooth transition for patients moving from the hospital environment back into community-based care programs. The coordinator acts as a liaison, working closely with referral sources such as hospitals, Accountable Care Organizations (ACO), physician practices, payers, and case managers, helping to coordinate and complete admissions while educating hospital staff, physicians, and community organizations about care continuity. This ensures that patients experience seamless service across care settings.

The Post Acute Coordinator’s responsibilities involve extensive communication and collaboration with patients, their families, healthcare personnel, and external agencies to coordinate care plans and discharge processes effectively. Managing patient information related to home health, hospice care, and PACE services is crucial for addressing reimbursement and service questions, supporting patients and staff to navigate care challenges. Additionally, coordinating physicians' orders or referrals with intake teams ensures timeliness and accuracy for patient admissions.

The employment position is Monday through Friday, from 8:00 a.m. to 5:00 p.m., with no evenings, weekends, or holidays required, which supports a balanced work-life environment. However, the role does require up to 50 percent travel to effectively connect with patients, families, and healthcare partners in various community settings.

Candidates should possess a bachelor’s degree in health promotion, marketing, sales, or a related field, or alternatively have significant relevant experience in healthcare sales and marketing. Familiarity with insurance processes and a valid driver’s license are also required, given the travel involved. Job Requirements

  • Bachelor's degree or equivalent experience
  • 5 years relevant experience if no degree
  • 2 years health care related sales and marketing experience
  • 1 year experience with frail or elderly population
  • Auto insurance

  • Ability and willingness to travel up to 50 percent

Job Qualifications

Job Duties

  • Communicate with referral sources including hospital, ACO, physician practices, payers, case managers to gain necessary information for patient care
  • Collaborate with patients, families and healthcare personnel or outside agencies regarding coordination of care and discharge planning
  • Manage information related to home health, hospice or PACE programs, services and reimbursements
  • Provide support or clarification to patients or staff as issues arise
  • Coordinate initial physicians' orders and/or referrals with intake teams
  • Identify and recommend utilization of other community resources when appropriate
  • Assist and educate patients, families, physicians, and clinicians with questions, clinical issues or other problems to facilitate transition to home, hospice, or PACE care
