Job Description

Dedicated Senior Medical Center, a ChenMed Company, is a nationally recognized leader in value-based primary care specializing in serving senior patients. Founded with the mission to provide concierge-style medicine to underserved elderly populations, ChenMed operates more than 50 senior medical centers across nine U.S. markets. ChenMed is a fully capitated Medicare Advantage provider, offering high-quality medical care with minimal to no out-of-pocket costs for beneficiaries. This innovative approach focuses on preventive care, better health outcomes, and comprehensive support services, all designed to enhance the lives of seniors who most need attentive and personalized medical care.

ChenMed is currently expanding its Philadelphia presence with the opening of three new offices in summer 2020, and due to ongoing success and growth, the company is seeking highly qualified Internal Medicine physicians, Family Medicine doctors, and Geriatricians to join their dedicated team. The company’s primary care practice model embraces full risk and accountability for patient outcomes, positioning primary care physicians (PCPs) as the central coordinators of all care management for their patients. Physicians work closely within a care team that includes nurses, case management social workers, a transitional care team, full in-house consulting specialists, and additional support personnel, ensuring that every patient receives well-rounded care.

Physicians at ChenMed enjoy a supportive and collaborative work environment that emphasizes manageable patient panel sizes, allowing meaningful patient interactions and care. Each physician is responsible for a panel of approximately 400 patients, facilitating more personalized and frequent visits (on average, patients are seen once per month). The practice offers a traditional Monday through Friday schedule with no night or weekend shifts, making work-life balance a priority. Call responsibilities are limited, telephonic, and scheduled in brief 2 to 3 week blocks throughout the year.

Job Requirements

  • Medical degree from an accredited institution
  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics
  • Active Pennsylvania medical license
  • At least one year of clinical experience preferred
  • Proficiency in electronic medical records (EMR) systems
  • Strong communication and interpersonal skills
  • Ability to work collaboratively with a diverse care team

Job Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
  • Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics
  • Valid medical license to practice in Pennsylvania
  • Experience or strong interest in senior patient care
  • Ability to work collaboratively in a multidisciplinary team
  • Commitment to value-based care principles

Job Duties

  • Manage a full patient panel and coordinate all aspects of primary care
  • Collaborate with care team including nurses, case managers, and social workers
  • Provide preventive care and chronic disease management for senior patients
  • Utilize in-house consulting specialists and transitional care teams for comprehensive patient management
  • Engage in telephonic practice call during scheduled blocks
