Dedicated Senior Medical Center, a ChenMed Company, is a prominent leader in senior-focused primary care with a mission to provide concierge-style medicine and improved health outcomes for the most vulnerable senior populations. Founded with the goal of making high-quality health care accessible to seniors most in need, ChenMed operates more than 50 senior medical centers across nine U.S. markets. As part of their continued expansion, ChenMed is growing their team in Cleveland, Ohio, where three new offices will open in the summer of 2020. They are actively seeking highly qualified Primary Care Physicians including Internal Medicine, Family Medicine, and Geriatricians who are passionate about improving the lives of senior patients. Physicians who join ChenMed in Cleveland will be part of an innovative practice model that places them at the core of patient care as general managers of their patients' health. The practice emphasizes collaboration among medical staff and support teams including nurses, case managers, and social workers, allowing physicians to focus on delivering personalized care. The model incorporates a comprehensive transitional care team with access to in-house consulting specialists, enhancing the care continuum and patient outcomes.
Physicians at ChenMed benefit from several unique features that improve both their professional and personal lives. They maintain small patient panel sizes—around 400 patients—which enables them to spend quality time with each patient, typically seeing them once per month. The schedule promotes work-life balance with a Monday to Friday work week, no nights or weekends, and limited telephonic call responsibilities conducted in brief blocks throughout the year. Additionally, doctors have access to an on-site lab and prescription dispensary, simplifying the care process. Job Requirements
- Valid medical license to practice in Ohio
- Completion of residency in Internal Medicine, Family Medicine, or Geriatrics
- Experience working with senior patient populations
- Ability to manage a panel of approximately 400 patients
- Willingness to work Monday through Friday with no nights or weekends
- Capacity to participate in telephonic call rotations
- Ability to engage in ongoing professional development
Job Qualifications
- Medical degree from accredited institution
- Board certification in Internal Medicine, Family Medicine, or Geriatrics
- Experience in primary care with a focus on senior patients preferred
- Strong communication and interpersonal skills
- Ability to work collaboratively within interdisciplinary teams
- Commitment to value-based, preventive care models
Job Duties
- Provide comprehensive primary care to senior patients
- Coordinate patient care as the primary general manager
- Collaborate with dedicated nurses, case managers, and social workers
