They are actively seeking highly qualified Primary Care Physicians including Internal Medicine, Family Medicine, and Geriatricians who are passionate about improving the lives of senior patients. Physicians who join ChenMed in Cleveland will be part of an innovative practice model that places them at the core of patient care as general managers of their patients' health. The practice emphasizes collaboration among medical staff and support teams including nurses, case managers, and social workers, allowing physicians to focus on delivering personalized care. The model incorporates a comprehensive transitional care team with access to in-house consulting specialists, enhancing the care continuum and patient outcomes.
