Job Description
Concierge Home Care is a reputable provider of home health services dedicated to offering personalized, high-quality care to patients within the comfort of their own homes. Located in Port St Lucie, Florida, this healthcare company specializes in delivering compassionate, patient-centered care by leveraging flexible scheduling and innovative technology to enhance the therapeutic experience. With a strong emphasis on work-life balance and professional autonomy, Concierge Home Care stands out as a leading employer for healthcare professionals seeking to grow their careers while delivering impactful service to their communities. They provide a supportive and collaborative environment that fosters growth through mentorship, ongoing education, and opportunities for career advancement.
The role of Home Health Physical Therapist (PT) at Concierge Home Care is designed for driven and compassionate licensed physical therapists who desire a flexible work schedule, autonomy in managing their caseload, and the opportunity to form meaningful patient relationships. This position is based in Port St Lucie and covers the surrounding St Lucie County, with territories organized to minimize travel time and optimize scheduling efficiency. The employment opportunity offers a pay-per-visit compensation model ranging from $70 to $90 per visit, plus additional pay based on Medicare differentials. Full-time therapists in this role have the potential to earn between $95,000 and $120,000 annually, with top performers exceeding $110,000 per year. Complimentary quarterly bonuses, mileage reimbursement, and a data plan stipend further enhance the total compensation package.
As a Home Health Physical Therapist, you will provide one-on-one therapeutic care in patients' homes, creating and executing individualized treatment plans to aid their recovery and improve mobility. This role involves evaluating patients, delivering therapeutic exercises, educating patients and caregivers about safety and recovery protocols, and collaborating closely with physicians and interdisciplinary teams to ensure comprehensive care. Additionally, you will supervise Physical Therapist Assistants (PTAs), utilize AI-assisted charting technology for streamlined documentation, and benefit from structured onboarding and continuous professional development, including free CEU courses. The role demands a genuine desire to improve patient well-being coupled with the professionalism to work independently while maintaining consistent patient volume provided by Concierge Home Care.
This position respects work-life balance by eliminating the need for weekend or holiday shifts (offered as optional flexibility) and avoiding hospital rotations and lengthy 12-hour shifts. With the support of user-friendly EMR systems like WellSky/Kinnser and AI-enhanced charting, you'll spend less time on paperwork and more time on direct patient care. If you are a Florida licensed physical therapist seeking to advance your career in a flexible, supportive, and technologically enabled home health care setting, Concierge Home Care offers an outstanding opportunity to practice at the top of your license while maximizing your earning potential and professional satisfaction.
