The opportunity at Heartland Home Health is for a Home Health Physical Therapist who plays a vital role in improving patients' mobility, strength, balance, and overall physical function. This position offers a competitive salary, including a $5000 sign-on bonus and the potential availability of a company car. As a Home Health Physical Therapist, you will deliver skilled physical therapy tailored to each patient’s individualized Plan of Treatment developed in collaboration with referring physicians. You will evaluate patient needs, develop rehabilitation goals, and adjust treatment plans to optimize outcomes. Collaborating closely with an interdisciplinary team consisting of nurses, occupational therapists, speech therapists, and social workers is essential to providing coordinated and comprehensive care.

You will also educate patients and their caregivers on exercises, safety measures, mobility strategies, and the use of assistive devices to enable them to maximize their independence. Accurate and timely documentation of visits, assessments, and progress notes is a critical part of this role. This position requires the ability to work independently across various home environments, delivering compassionate and effective therapy services to patients of diverse backgrounds and conditions. Job Requirements

  • Master's degree in Physical Therapy
  • Current state-issued physical therapist license or eligibility
  • CPR certification
  • Strong communication, assessment, and problem-solving skills
  • Ability to work independently in home environments
  • Valid driver's license
  • Auto insurance
  • Reliable transportation
