Job Description
VNA Health at Home is a prominent healthcare provider delivering comprehensive home health care services to patients in their own homes. As part of a wide-reaching health care network, VNA Health at Home is deeply committed to improving patient outcomes by providing high-quality care in the most comforting and functional environment—their home. This faith-based organization places a strong emphasis on humankindness, ensuring that every aspect of care is rooted in compassion, respect, and personalized attention. The mission of VNA Health at Home expands beyond individual patient care to encompass the well-being of families and the broader communities it serves, making it a respected leader in the healthcare industry.
The organization is currently seeking a dedicated and skilled Physical Therapist to join its team. This role is pivotal in delivering patient-centered rehabilitation services directly in patients' homes, where functional movement improvement takes on the most practical and meaningful context. The Physical Therapist will serve as a crucial member of an interdisciplinary team that collaborates closely with patients, families, caregivers, and healthcare professionals to develop and implement personalized treatment plans tailored to each patient's unique physical capabilities and recovery goals.
This is a full-time, hourly position with a competitive pay range of $40.95 to $60.91 per hour, along with a $5,000 sign-on bonus incentive. The Physical Therapist's core responsibilities include conducting thorough evaluations of patients' physical abilities, mobility, strength, and functional limitations, and applying therapeutic exercises, manual therapy, and modalities aimed at enhancing mobility and physical function. Educating patients and their families on safety, exercises, and lifestyle modifications is also a significant component of the role, helping to promote long-term recovery and prevent re-injury.
Furthermore, this position requires careful monitoring and documentation of patient progress, with adjustments to treatment plans as necessary to ensure optimal outcomes. Accurate maintenance of patient records in compliance with state regulations and agency policies is mandatory. The home care setting enables the Physical Therapist to impact patients where they spend most of their time, reinforcing the practicality and effectiveness of rehabilitation efforts in the home environment.
By joining VNA Health at Home, the successful applicant will benefit from a supportive and rewarding work environment that values professional development and employee well-being. The organization offers a comprehensive benefits package including competitive bonus structures, generous vacation policies with paid holidays and personal days, comprehensive medical, dental, and vision insurance plans, tuition reimbursement for continued education, and participation in a Fidelity 401(k) retirement plan. The commitment to providing compassionate care and professional growth makes this an ideal opportunity for Physical Therapists looking to make a meaningful difference in patient lives while advancing their careers in a valued healthcare organization.
