The Patient Navigator role within the Health System Shared Services Department Patient Facilitated Services program is a pivotal position at The Ohio State University Wexner Medical Center. The Patient Navigator is responsible for delivering outstanding customer service by managing and coordinating care for targeted patient populations in an expedited, personalized, and innovative manner. Serving as a vital link between patients and the medical center, the Patient Navigator supports individuals before, during, and after their encounter with the health system to ensure seamless access to services. This position demands a high level of interpersonal skill, a strong service orientation, and the ability to handle complex healthcare navigation tasks effectively.

The Patient Facilitated Services program aims to maintain and deepen relationships with those who actively support the mission of Ohio State and to create beneficial connections that impact patients' lives positively. The Patient Navigator acts as a coordinator of the healthcare concierge program, facilitating ambulatory care, patient visits, and addressing various patient needs with excellence in service delivery. Additionally, the role involves participating in the entire spectrum of Patient Facilitated Services responsibilities, transferring knowledge within the team, and leveraging staff resources to optimize patient engagement.

Strategically, the Patient Navigator position serves multiple functions beyond patient care coordination; it is intricately linked with Ohio State’s business and fundraising initiatives. This role liaises with the Ohio State Office of Development to manage grateful patient stewardship at the principal and major donor levels, and collaborates with the Office of Government Relations along with other external offices. This ambassadorial position represents Ohio State’s commitment to exceptional service and relationship management, acting as a key contact for unique patient audiences and contributing to the university’s overarching goals.

The role is based at University Hospital - Doan Hall, involves a regular full-time schedule with varying shifts, and requires a candidate ready to engage in a dynamic and rewarding healthcare environment. Candidates will be required to complete a background check and may need to undergo a drug screen or physical examination after an offer is made. Job Requirements

  • Bachelor's degree or equivalent experience
  • 2-4 years of relevant experience preferred
  • Knowledge of academic institutions and/or academic medical centers
  • Strong appreciation for and ability to consistently provide the highest level of customer service
  • Ability to work varying shifts
  • Successful completion of background check
  • Drug screen or physical may be required during post offer process

Job Qualifications

  • Bachelor's degree or equivalent experience
  • 2-4 years of relevant experience preferred
  • Knowledge of academic institutions and/or academic medical centers

  • Ability to work collaboratively within multidisciplinary teams
  • Experience in healthcare navigation or patient advocacy preferred

Job Duties

  • Coordinate and manage care for targeted patient populations in an expedited and personalized manner
