Job Description
Suncoast Hospice, a respected part of Empath Health, has been providing compassionate end-of-life care since 1977 across Pinellas and Hillsborough counties in Florida. As a nonprofit healthcare organization, Empath Health offers a connected network of services including hospice, home health, grief care, geriatric primary care, elder care, HIV and sexual health care, and dementia support. Suncoast Hospice is currently seeking a dedicated Patient Concierge (Care Navigator) to join their compassionate team. This role serves as a first responder for the agency, acting as an essential liaison between the patient, family members, authorized representatives, and other stakeholders to ensure seamless access to hospice services. The Patient Concierge embodies patient-centered care, placing the needs and well-being of the patient and their families at the forefront. They exceed customer expectations by providing attentive, knowledgeable, and empathetic service with flawless care throughout every step of the process.
Scheduled Monday through Friday from 9 am to 6 pm, the Patient Concierge works closely with the interdisciplinary team across various care settings, including traveling to the patient when necessary. Their responsibilities include documenting patient admissions accurately and in real time, educating patients and families about advance directives, and navigating Florida proxy guidelines to ensure all admission documentation complies with legal standards. This role also involves being an ambassador for Empath Health, promoting the organization's values of kindness, compassion, and full life care in every customer interaction.
The ideal candidate is a detail-oriented and compassionate professional with excellent communication skills, knowledge of medical terminology, and strong computer proficiency. They must understand hospice programs and community resources and be familiar with the reimbursement processes for hospice services. Reliable transportation and the ability to travel throughout the designated counties are required to perform timely and effective patient visits. Job Requirements

Job Duties
- Completes required documentation in real time by being accurate, thorough and detail-oriented from referral through admission
- work alongside the interdisciplinary team in all settings, while traveling to where the patient is
- assist with inquiries and education regarding self-determined life closure issues (Advance Directives) and ensure proper signage of all forms related to admission process
- act as an ambassador for Empath Health when communicating with all customers
- participate and promote agency strategic goals related to access, customer service and quality
- consistently update knowledge of Empath Health programs
