The Patient Care Coordinator-Infections Disease role at Arcare is a full-time position with working hours from Monday to Friday, 8:00 AM to 5:00 PM. This role is pivotal in ensuring seamless care coordination and patient advocacy for clients living with infectious diseases, primarily focusing on HIV/AIDS. The Patient Care Coordinator will act as the primary liaison between patients, their families, healthcare providers, and community resources.
