The Patient Care Coordinator-Infections Disease role at Arcare is a full-time position with working hours from Monday to Friday, 8:00 AM to 5:00 PM. This role is pivotal in ensuring seamless care coordination and patient advocacy for clients living with infectious diseases, primarily focusing on HIV/AIDS. The Patient Care Coordinator will act as the primary liaison between patients, their families, healthcare providers, and community resources. Their responsibilities include managing care plans, facilitating communication among stakeholders, and connecting patients with essential services such as behavioral health, insurance navigation, and coordinated care programs. This role requires a strong understanding of healthcare systems, insurance policies, and available community resources, which will be crucial in helping patients navigate their treatment and support options effectively. The coordinator will also be responsible for scheduling, tracking patient progress, and identifying potential gaps in care to improve patient outcomes continually.
This position requires an individual with strong organizational and communication skills, capable of collaborating effectively with multidisciplinary teams. The person should be committed to patient advocacy and able to ensure compliance with healthcare regulations and standards. Job Duties
- Oversee the coordination of patients care while serving as a primary contact for patient and their families
- Assist with scheduling, transportation, and connecting patients to community resources and support programs
- Track patient progress, while identifying and resolving care coordination gaps to enhance patient outcomes
- Participate in ongoing training related to care coordination and quality
- Facilitate communication between patients, families, healthcare providers, and community stakeholders
- Manage care plans and provide referrals to behavioral health and insurance services
- Advocate for patient needs and ensure compliance with healthcare standards
