The Patient Care Coordinator (PCC) plays a vital role within Kaiser Permanente, specifically in managing and coordinating care for the acute inpatient population. This position is essential in ensuring seamless continuity of care and promoting optimal health outcomes for patients transitioning through the hospital system. The PCC collaborates closely with hospital-based service physicians, nursing staff, utilization review, social workers, and ambulatory case managers, along with patients and their families, to develop and implement personalized discharge plans that emphasize safe discharge and prevent readmission.

This role requires the PCC to perform timely and comprehensive face-to-face assessments of admitted patients within 24 hours, identify discharge needs, and document findings accurately in the electronic medical record system. Active participation in daily Care Without Delay rounds is mandatory to communicate patient progress and address barriers to discharge effectively. The PCC must demonstrate strong organizational and critical thinking skills to manage and expedite care progression while coordinating referrals for post-acute care services such as home health, durable medical equipment, sub-acute rehabilitation, and skilled nursing facilities.

Beyond clinical coordination, the PCC must also uphold facility safety policies, regulatory requirements, and quality assurance initiatives to contribute to continuous improvement efforts. This position calls for excellent communication and interpersonal abilities to work collaboratively with multidisciplinary teams and interact empathetically with patients and their families. Job Requirements

  • Minimum of one year experience in an acute medical/surgical/ED or critical care nursing area
  • Associate nursing degree required
  • Credentials from the primary work state are required at hire
  • Registered Nurse License (Virginia) within 6 months of hire and Registered Nurse License (Maryland) within 6 months of hire or Compact License - Registered Nurse within 6 months of hire and Registered Nurse License (District of Columbia) within 6 months of hire
  • Basic Life Support
  • Experience using an electronic medical record system
  • Some awareness or knowledge of healthcare reimbursement systems (Medicare Advantage, Commercial payer, Medicaid and CMS regulatory rules)
  • Successful completion of PCC Assessment of Critical Skills annually, passing score on inter-rater reliability
  • Ability to effectively communicate with physicians, members and their family or representatives, and hospital staff
  • Ability to work independently and apply critical thinking skills for problem solving and decision making
  • Adheres to KP Employees Handbook and facility policies and procedures
  • Excellent time management skills to develop organized work processes in a high-volume environment with rapidly changing priorities
  • Intermediate computer skills (Microsoft Office Suite proficiency)
  • Ability to tolerate and cope with ambiguity

  • Excellent verbal and written communication skills

Job Duties

  • Complete an initial face-to-face assessment for every admitted member to identify discharge needs within 24 hours of admission
  • Document in KPHC and communicate assessment outcomes to determine the appropriate transition plan with MAPMG physician healthcare team and patient/family
  • Actively participate in daily Care Without Delay rounds reporting on patient progression towards the established discharge plan
  • Review and document discharge plan in accordance with KP discharge planning documentation policies, facility policies, and regulatory requirements
  • Document any updates, care progression, and barriers to discharge daily and as indicated on assigned patients
  • Manage timeliness of care progression with physician and nursing staff to prevent avoidable delays or days
  • Collaborate with Social Worker to coordinate long-term care, assisted living, financial assistance, and other services as required
