The Patient Care Coordinator role at Optum is a vital position that serves as an ambassador for the organization, ensuring that patients and providers receive coordinated, high-quality care. This full-time, field-based position requires extensive travel within Pasco County, Florida, and surrounding communities, as well as close collaboration with interdisciplinary primary care teams including nurse practitioners and physician assistants. The Coordinator’s primary responsibilities include entering data into electronic medical records (EMR), managing member charts, and obtaining medical records from hospitals and physician offices to support post-hospital care transitions and ongoing patient management activities.
In this role, the Patient Care Coordinator not only handles administrative tasks efficiently but also plays a crucial role in coordinating care services such as scheduling patient appointments and arranging transportation. Flexibility, attention to detail, and the ability to manage shifting priorities are essential qualities, along with a commitment to protecting sensitive patient information. Working hours are Monday through Friday, 8:00 am to 5:00 pm, with no night, weekend, or holiday shifts, offering a stable work schedule alongside the robust benefits and training provided by Optum.
Compensation for this position ranges from $20 to $36 per hour, depending on experience, education, and certifications. Job Requirements
- High school diploma or GED or equivalent work experience
- Valid driver’s license in State of Florida
- At least 18 years of age
- Minimum 3 years of clinical or medical record experience
- Access to reliable transportation for travel
- Ability to travel up to 100% of the time
- Residence within Pasco County, FL or surrounding communities
- Ability to work full-time Monday through Friday from 8:00 am to 5:00 pm
- Must maintain a dedicated work area that provides information privacy
- Must live in an area with approved high-speed internet
- Pass a drug test prior to employment
Job Qualifications
- High school diploma or GED or equivalent work experience
- Minimum 3 years of clinical or medical record experience
- Knowledge of medical terminology and electronic health records platforms
- Ability to travel extensively within Pasco County, FL and surrounding communities
- Valid driver’s license in the State of Florida
- Ability to work full-time Monday through Friday from 8:00 am to 5:00 pm
- Strong organizational and communication skills
- Ability to manage shifting priorities and attention to detail
- Preferred certifications such as certified nursing assistant or medical assistant
- Ability to maintain confidentiality of sensitive information
- Reliable access to a UnitedHealth Group approved high-speed internet connection or existing high-speed internet
Job Duties
- Support interdisciplinary care team with overall member coordination at the direction of RN or NP/PA
- Data entry and member chart management for new members, post hospital visits, quality, labs and medications
- Obtain medical records following hospitalizations or physician appointments
- Scheduling member transportation or appointments
- Provide operational support including backfill and care coordination activities
- Collaborate and communicate with SCC members within the team model
- Support nurse practitioners, physician assistants, and patients through administrative tasks and patient care coordination
