The Activities Coordinator position at UHC plays a vital role in leading participant engagement within the Program of All-Inclusive Care for the Elderly (PACE) Center. This full-time, hourly role starts at $29.28 per hour, with salaries dependent on knowledge, skills, and experience. The Activities Coordinator is responsible for organizing, planning, and facilitating a broad range of social, recreational, and therapeutic activities designed to encourage socialization and promote cognitive and physical well-being among participants. Working collaboratively within a multidisciplinary team, this role demands creativity, flexibility, and strong communication skills to cater to the individual interests and needs of each participant. The coordinator is entrusted with planning monthly calendars, conducting assessments, and tailoring activities to accommodate dementia-specific populations and other special needs.

Key responsibilities include managing community outings, small group sessions, and providing leisure education to participants and caregivers. The position also involves substantial organizational work such as maintaining activity areas, managing supplies, overseeing budgeting, and documenting participant progress. In addition to direct participant interaction, the Activities Coordinator supports Recreation Assistants, volunteers, and team members with program development and dementia-related care strategies. This role demands strong reliability, organizational competency, and confidentiality adherence to align with regulatory standards and organizational policies.

Job Qualifications

Job Duties

  • Plans a variety of activities including exercise, arts and crafts, celebrations, special events, recreational games, community outings, and cognitive groups
  • organizes community outings and special events at Adult Day Health Center including arranging transportation, entertainment, catering, and staff
  • develops and distributes a monthly calendar of activities
  • plans and implements small group or one-on-one activities for dementia-specific population including sensory stimulation and oversees daily flow of dementia activities area
  • assists participants and caregivers by providing leisure education for activity modification or skill development and supplies for in-home use
  • functions as a participant of the interdisciplinary team, attends team meetings, and reports changes in participants' baseline status
  • maintains general order in activities area including daily clean-up, supply inventory, ordering, and purchasing, and manages budget
  • assesses involvement in activity for participants placed in group or nursing homes and collaborates with facility staff
  • completes documentation such as assessments and individual service plans
  • provides direction and support to Recreation Assistants, volunteers, and other staff
  • offers ongoing support to staff involved in dementia programming
  • transports participants within the ADHC and rearranges activity rooms as needed
  • maintains and promotes compliance with regulatory, reimbursement, organizational standards, and policies
  • completes annual competency requirements
  • consistently achieves high standards of accuracy in work and documentation
  • practices universal precautions and safety techniques
  • protects privacy and maintains confidentiality
  • attends and participates in meetings and activities
  • attends workshops/seminars to increase skills
