The role of Home Health Occupational Therapist at CenterWell Home Health is essential to this mission. This position involves providing skilled occupational therapy services to patients in their home environments with a focus on regaining functional abilities and promoting independence. Reporting to the Home Health Branch Administration, therapists will assess patients' daily living and work-related skills, develop therapeutic retraining programs with measurable goals, and administer diagnostic and prognostic tests to assist physicians in evaluating patient function. They collaborate closely with the healthcare team and participate actively in developing and reviewing treatment plans.

Home Health Occupational Therapists provide therapy designed to improve or restore strength, coordination, range of motion, and functional capabilities, often adapting therapeutic programs based on patients’ progress and behavioral goals. They are instrumental in coordinating with vocational teams when necessary, and in recommending or designing adaptive equipment to assist patients in overcoming environmental and physical challenges. Therapists also design community reintegration activities, training patients, family members, and staff on therapeutic techniques related to daily activities such as dressing, feeding, grooming, and hygiene. Supervisory duties may include overseeing the performance of Occupational Therapy Assistants and Home Health Aides involved in implementing therapy services.

The role requires accurate and thorough documentation of patient care, observations, interventions, and progress, with strict adherence to physician orders and compliance with regulatory standards. Additionally, therapists participate in care coordination, discharge planning, and act as a resource to the healthcare team to meet patient needs effectively.
