The current opportunity is with Optage Hospice, a division of PHS, where they are looking to hire a Full-Time Floating Nursing Assistant/Home Health Aide. This role uniquely requires flexibility as the position involves floating to various regions within the Twin Cities metro area, including locations in Wisconsin such as Hudson and New Richmond. The Floating Nursing Assistant/Home Health Aide will provide personalized, one-on-one care to hospice patients in a variety of settings including private residences, group homes, assisted living, or skilled nursing facilities. This is a full-time position with weekday hours (Monday through Friday, 7 a.m. to 2:30 p.m.), with no weekend shifts required. The role offers between 8 and 10 shifts per pay period, catering well to individuals seeking consistent yet flexible scheduling.

Candidates must have completed a state-approved nursing assistant training program and hold current certification, or be willing to obtain Wisconsin certification within three months of employment. The employer reimburses mileage and covers paid travel time to facilitate the floating nature of this role. Importantly, prior active registration with the Minnesota Nursing Assistant Registry is not required. Job Requirements

  • High school diploma or equivalent
  • Completion of state-approved nursing assistant training
  • Current nursing assistant certification or willingness to obtain within 3 months
  • Ability to float between service regions as needed
  • Own transportation with willingness to travel to different locations including out-of-state sites
  • Strong communication and interpersonal skills

Job Qualifications

  • Successful completion of a state-approved nursing assistant training program
  • Current certification in the state(s) of practice or willingness to obtain Wisconsin certification within three months
  • Demonstrated competencies in person-directed, elder/resident-directed practice
  • Ability to work effectively as part of a team and demonstrate interpersonal effectiveness
  • Strong critical thinking and self-leadership skills

Job Duties

  • Provide individualized, one-on-one care to hospice patients in home and community settings
  • Work under the supervision of a Registered Nurse to meet clients' personal needs and promote comfort
  • Float to various regions within the Twin Cities metro area and parts of Wisconsin including Hudson and New Richmond
  • Assist with activities of daily living following established regulations and best practices
  • Maintain accurate records of care provided and report changes in patient condition
  • Support interdisciplinary healthcare team with communication and coordinated care efforts
  • Adhere to safety standards and infection control protocols
