The Primary Care Provider (PCP) Nurse Practitioner role at ChenMed is an extraordinary opportunity for healthcare professionals dedicated to making a tangible difference in patient lives while advancing their careers within a supportive and dynamic environment. The PCP is held accountable for clinical outcomes and cost-effectiveness for a panel of up to 450 patients, emphasizing preventive care, patient coaching, and behavioral health changes that support long-term wellness. This role goes beyond traditional sick care by emphasizing continuous patient engagement, coaching, and goal-setting to deliver superior health results.

In this outcome-driven, value-based organization, PCPs champion quality, patient satisfaction, and clinical excellence. Metrics routinely tracked include patient admissions, the number of appointment slots utilized, patient experience scores, clinical gap closures, and medical cost management. The PCP maintains a leadership role within their care team, collaborating with care promoters, facilitators, and coordinators, and ensures seamless care coordination for both office-based and housebound patients. They engage actively with hospitalists and transitional care teams to optimize patient management during hospital stays or other care transitions.

Job Qualifications

  • Current nurse practitioner certification in the state of practice
  • certification in Basic Cardiac Life Support
  • board certification by AANP or ANCC preferred or required depending on state
  • current DEA number for schedule II-V controlled substances if required
  • proficiency in Microsoft Office Suite
  • fluency in English verbal and written
  • willingness to travel locally, regionally, and nationally for training and recruiting purposes

Job Duties

  • Independently provide care for patients with acute and chronic illnesses in older adults
  • take full accountability for patient care and outcomes
  • engage with hospitalists when patients are hospitalized
  • perform assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation
  • lead a care team including care promoters, facilitators, and coordinators
  • collaborate with transitional care teams for patients unable to visit the office
  • contribute to center management and provider coverage
  • supervise and collaborate with Advanced Practice Practitioners
  • perform additional duties as assigned
