Empatia is actively seeking a compassionate and skilled Palliative Care Nurse Practitioner to join their interdisciplinary team. This advanced practice role involves providing patient-centered palliative care in home settings, working autonomously in collaboration with the Palliative Medical Director. The successful candidate will be responsible for comprehensive patient assessments, developing individualized care plans, managing treatment adaptations, prescribing medications—including controlled substances—and maintaining accurate clinical documentation. This role demands expertise in palliative medicine principles, home health and hospice care processes, along with familiarity with Medicare and Medicaid regulations.

The Palliative Care Nurse Practitioner will play a critical role in guiding patients and families through the physical, emotional, psychosocial, and spiritual challenges of advanced illness. Strong communication skills and teamwork are essential, as is the ability to interact effectively with physicians, caregivers, and other health professionals. The position also involves participating in evening and weekend call rotations and facilitating additional care service referrals as appropriate.
