We are currently seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our interdisciplinary care team. In this pivotal role, you will autonomously provide advanced palliative care services to patients within their homes, ensuring they achieve the highest possible quality of life while managing complex medical conditions. You will collaborate closely with the Palliative Medical Director and other healthcare professionals to deliver comprehensive, patient-centered care. This position demands an individual who is self-motivated and equipped with the necessary skills to perform thorough patient assessments encompassing physical, emotional, and psychosocial dimensions.

Your responsibilities will include developing personalized care plans in partnership with patients and their families, managing and adjusting treatment strategies based on patient progress, prescribing medications (including controlled substances subject to state regulations), and ordering diagnostic tests as needed. Additionally, you will maintain meticulous clinical documentation in adherence to professional standards, facilitate communications among care team members, and participate in evening and weekend call rotations to ensure continuous patient support.

The ideal candidate possesses excellent leadership abilities, interpersonal skills, and communicates effectively both verbally and in writing. A profound understanding of palliative medicine principles, eligibility criteria for home health and hospice care, as well as Medicare and Medicaid regulations is essential. Familiarity with the interdisciplinary care process and proficiency with point-of-care software and technology are also required. Academic qualifications include a Master of Science in Nursing or its equivalent at the state's discretion, with a minimum of two years of Nurse Practitioner experience, preferably specializing in Family or Adult Medicine. Candidates should have experience with Part B billing and the capability to operate independently in an advanced practice role.

Licensure and certifications required for this role include a current RN license, Nurse Practitioner certification or license aligned with state mandates, collaborative agreements with supervising physicians, Part B Provider certification, and valid DEA licenses as applicable. Holding ACHPN certification in Hospice and Palliative Care and possessing a valid driver's license with current automobile insurance are preferred prerequisites.
