We are currently seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our healthcare team. This full-time role offers an exciting opportunity to deliver direct primary care services within assisted and independent living communities. The ideal candidate will be responsible for conducting comprehensive patient assessments, managing chronic conditions, promoting wellness, and coordinating care with other healthcare professionals.
