We are currently seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our healthcare team. This full-time role offers an exciting opportunity to deliver direct primary care services within assisted and independent living communities. The ideal candidate will be responsible for conducting comprehensive patient assessments, managing chronic conditions, promoting wellness, and coordinating care with other healthcare professionals. As part of our team, you will perform routine rounds to monitor residents’ health, adjust treatment plans as necessary, and respond promptly to emergent medical needs. Education plays a crucial role in this position, as you will provide guidance to residents, families, and facility staff on disease prevention and chronic illness management to support ongoing wellness. Additionally, the role involves maintaining meticulous documentation within electronic medical record (EMR) systems, adhering to regulatory standards and organizational protocols, and contributing to quality improvement initiatives that reduce hospital readmissions and increase patient satisfaction.

Job Requirements
- Licensed nurse practitioner or physician assistant in state of operation
- Current board certification
- Masters degree in nursing or physician assistant studies
- Ability to travel to assigned facilities
- Excellent communication and interpersonal skills
- Strong organizational and teamwork abilities
- Valid driver’s license, auto insurance, and acceptable driving record may be required with notice
Job Qualifications
- Valid nurse practitioner or physician assistant license in state of operations
- Current board certification in your field
- Masters degree in nursing or physician assistant studies
- Strong leadership, interpersonal, and problem-solving skills preferred
- Excellent written and oral communication skills
- Punctuality, organizational skills, and ability to work effectively within a team
Job Duties
- Perform comprehensive patient assessments, develop treatment plans, and manage ongoing care for residents
- Conduct routine rounds to evaluate residents’ health status and modify treatment as needed
- Collaborate with facility staff, family members, and other healthcare providers to ensure coordinated, patient-centered care
- Provide education on disease prevention, wellness, and chronic disease management to residents, families, and staff
- Maintain accurate and timely documentation in electronic medical records to comply with regulatory standards
- Participate in quality improvement initiatives to enhance care delivery and reduce hospital readmissions
- Ensure patient safety and satisfaction through proactive health monitoring and intervention
