Job Description

Curana Health is a leading healthcare company specializing in value-based care, specifically tailored to senior living communities and skilled nursing facilities. Founded in 2021, Curana Health has rapidly expanded to serve over 200,000 seniors across more than 1,500 communities in 32 states, supported by a dedicated team of over 1,000 clinicians along with care coordinators, analysts, and operational professionals. Ranked #147 on the Inc. 5000 list of America’s fastest-growing private companies and recognized within the healthcare and medical industry categories, Curana Health is committed to radically improving the health, happiness, and dignity of older adults through innovative healthcare solutions.

At Curana Health, the focus is on delivering high-quality primary care services that promote wellness, manage chronic conditions, and enhance overall health outcomes for seniors living in assisted and independent living communities. The company offers a comprehensive range of solutions including on-site primary care, Accountable Care Organizations (ACO), and Medicare Advantage Special Needs Plans, all designed to streamline operations for senior living operators while creating new financial opportunities. The role of Nurse Practitioner or Physician Assistant within Curana Health is both rewarding and impactful, requiring compassionate healthcare professionals who are dedicated to making a meaningful difference in the lives of older adults.

The Nurse Practitioner or Physician Assistant will provide direct patient care in a senior living setting, focusing on comprehensive assessments, developing and managing treatment plans, and coordinating care with facility staff, family members, and other healthcare providers. This role entails conducting routine rounds for ongoing monitoring, educating patients and caregivers on disease prevention and wellness, and maintaining detailed documentation to ensure compliance with regulatory standards. Additionally, the role contributes to quality assurance initiatives aimed at enhancing patient satisfaction and minimizing hospital readmissions.

Curana Health supports its healthcare team members by offering a highly autonomous practice environment backed by strong clinical and administrative support. The role benefits from cutting-edge care models that leverage data-driven tools and artificial intelligence to better understand and manage patient needs. Communication is streamlined with modern technology platforms, enabling efficient collaboration and coordination. Furthermore, the company is committed to professional development, providing continuing medical education (CME) support and quality incentives linked to patient outcomes and care excellence. Work-life balance is also emphasized with generous paid time off, reflecting Curana’s holistic approach to employee well-being.

This position requires occasional travel to various assigned senior living facilities, and depending on the travel requirements, may require maintaining a valid driver’s license, auto insurance, and an acceptable driving record. Curana Health values punctuality, organizational skills, teamwork, and effective communication as essential traits for success in this role. Overall, this is an outstanding opportunity for Nurse Practitioners or Physician Assistants who are passionate about transforming senior healthcare and eager to join a fast-growing, innovative company making a national impact.
