We are currently seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our dedicated healthcare team. This full-time role focuses on delivering exceptional primary care within senior living facilities including assisted living and independent communities. The selected candidate will be instrumental in promoting wellness, managing chronic conditions, and coordinating care in a manner that improves the health outcomes and overall quality of life for senior residents.

The Nurse Practitioner or Physician Assistant will perform comprehensive patient assessments, develop personalized treatment plans, and provide continuous care management emphasizing preventive care and chronic disease management. Routine rounds are conducted to monitor residents’ health, adjust interventions as needed, and promptly address emergent health concerns. The role requires close collaboration with facility staff, family members, and other healthcare providers to ensure coordinated and seamless care delivery tailored to individual patient needs.

Patient education is a critical component of the position, with responsibilities including educating residents, families, and staff on disease prevention, wellness strategies, and effective chronic illness management to foster improved health outcomes. Maintaining accurate and timely documentation in the electronic medical record (EMR) system is essential to ensure compliance with regulatory requirements and internal protocols. Moreover, the practitioner will actively participate in quality improvement initiatives designed to enhance care standards, reduce hospital readmissions, and increase patient satisfaction within the community.
