Job Requirements
- Current and unrestricted nurse practitioner or physician assistant license
- certificate of completion from an accredited Nurse Practitioner or Physician Assistant program
- current DEA certificate
- BLS certification
- minimum two years of clinical experience as an NP or PA
- experience in primary care or adult/geriatrics
- strong communication and interpersonal skills
- ability to perform physical and in-home assessments
- knowledge of HCC documentation and ICD-10 coding
- commitment to patient confidentiality and HIPAA compliance
Job Qualifications
- Must have or be eligible to have a current and unrestricted NP or PA license
- experience with risk adjustments and/or home assessments
- understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and Health Risk Assessments (HRAs)
- embrace teamwork and the opportunity to collaborate with talented colleagues
- certificate of completion from an accredited Nurse Practitioner Program or Physician Assistant Program
- current DEA certificate required prior to start date
- BLS certification
- at least two-year experience practicing as an N.P. or P.A.
- two years of experience in Primary Care or Adult/Geriatrics
Job Duties
- Evaluate and treat center patients in accordance with standards of care
- care coordination with other providers, specialists, and care facilities
- perform comprehensive in-home assessments for identified patients of the practice and assist the primary care team in better managing chronic diseases
- complete physical examinations, medication reviews, vital checks, lab draws, wound care and depression screenings
- communicate assessment findings that will be used to inform the PCP of potential gaps in care
- have support of Medical Assistants while seeing patients to allow more time for direct patient care
- educate patients on topics such as disease process, medication, and compliance
