The Nurse Practitioner, Advanced Practice Provider role at Oak Street Health represents a unique opportunity to make a significant impact on patient care through independent management of patient health alongside collaborative efforts with physicians and other healthcare professionals. Situated in Lansing, MI, this full-time position operates Monday through Friday from 8 am to 5 pm with a competitive salary range between $129,000 and $148,000, depending on experience and location.

In this role, Nurse Practitioners provide comprehensive primary care services focused on adults covered by Medicare. They are responsible for managing longitudinal care and developing enduring relationships with patients to foster trust and engagement in their health plans. This approach ensures patients are well-informed and connected to their care strategies, which supports better health outcomes overall.

Nurse Practitioners operate within an outpatient care setting, working as part of a multidisciplinary team consisting of physicians, nurse practitioners or physician assistants, medical assistants, medical scribes, registered nurses, and care managers. Oak Street Health further supports its providers with access to a robust network of specialists and hospitals for specialized and acute care needs. This team-based model enables healthcare providers to concentrate primarily on the delivery of primary care, care coordination, and population health management.

Beyond clinical responsibilities, Nurse Practitioners engage actively in population health initiatives such as ensuring evidence-based screenings are completed on schedule and participating in outreach and triage efforts. They also deliver patient education to empower individuals regarding their conditions, care plans, and treatments. Additionally, Nurse Practitioners at Oak Street Health may be involved in promotional activities and conduct home visits when necessary to extend care beyond the clinic.

This position reports directly to the Center Medical Director and collaborates closely with operational leadership to optimize care delivery. The role requires a commitment to excellence in primary care and a passion for teamwork within an innovative, team-oriented healthcare model aimed at empirically guided population health strategies. Job Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • national certification in at least one specialty: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner
  • active, non-probationary state Nurse Practitioner license
  • active DEA license
  • US work authorization

Job Duties
