The Nurse Practitioner, Advanced Practice Provider role at Oak Street Health is a full-time position with a schedule from Monday to Friday, 8:00 AM to 5:00 PM, and a salary range of $95,000 to $148,500, depending on experience and location. The role is pivotal in providing effective primary care services independently while also collaborating with physicians as needed. Nurse Practitioners at Oak Street Health are responsible for delivering longitudinal care, fostering meaningful patient relationships, and ensuring patients understand and remain connected to their health plans through education and guidance.
This role requires proficiency in conducting office visits for acute and routine health issues, administering annual wellness visits and health risk assessments with detailed and accurate documentation, coordinating care with other healthcare providers and agencies, and participating in population health leadership efforts. Nurse Practitioners at Oak Street Health also assist care teams with phone triage and outreach, participate in promotional health activities, and occasionally conduct home visits to ensure patient well-being. The role reports to the Center Medical Director and collaborates closely with operational leadership to maintain exceptional primary care standards.

Job Requirements
- Master of Science in Nursing or Doctor of Nursing Practice or NP Certificate Program graduate
- National certification in at least one approved specialty
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US work authorization
Job Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification as Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US work authorization
- Experience in primary care such as internal medicine, geriatrics, or family medicine preferred
- Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments preferred
- Passion for teamwork and cross-functional collaboration
- Desire to work in an innovative population health model
- Bilingual proficiency in applicable areas preferred
Job Duties
- Provision of exceptional primary care
- Conduct office visits for routine and acute issues
- Administer annual wellness visits and health risk assessments with accurate documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives in coordination with the care team
- Assist the care team with phone triage and outreach
- Educate patients on their health conditions, care plans, and treatments
- Participate in promotional health activities
- Conduct home visits as needed
- Perform other duties as assigned
