Job Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • national certification in at least one of the specified specialties
  • active, non-probationary state Nurse Practitioner license
  • active DEA license
  • US work authorization

Job Qualifications

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • national certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
  • experience in primary care including internal medicine, geriatrics, or family medicine preferred
  • understanding of HCC documentation, ICD-10 coding, and health risk assessments
  • passion for teamwork and cross-functional collaboration
  • desire to work in an innovative, empirically-guided population health model
  • bilingual proficiency preferred in applicable areas

Job Duties

  • Provision of exceptional primary care
  • conduct office visits for routine and acute issues
  • administer annual wellness visits and health risk assessments
  • care coordination with other providers, specialists, testing facilities, and agencies
  • population health leadership in coordination with the care team
  • assist the care team with phone triage and outreach
  • educate patients on their health conditions, care plans, and treatments
  • participate in Oak Street Health promotional activities
  • conduct home visits as needed
  • perform other duties as assigned
