As part of this mission, Oak Street Health is seeking a full-time Nurse Practitioner, Advanced Practice Provider, to join their outpatient practice team. This role operates Monday through Friday, from 8:00 AM to 5:00 PM, and is focused on providing exceptional primary care services to Medicare patients. The Nurse Practitioner will function both independently and as a crucial part of a collaborative care team that includes physicians, medical assistants, registered nurses, care managers, and medical scribes.

The Nurse Practitioner is entrusted with the responsibility of delivering comprehensive and equitable healthcare that keeps patients healthy while preventing unnecessary hospitalizations. Key responsibilities include conducting office visits for both routine and acute medical issues, performing annual wellness visits, and completing health risk assessments with meticulous attention to detail. This role also emphasizes patient education to ensure individuals understand their health conditions and care plans, fostering meaningful, trusted relationships.

In addition to clinical care, the Nurse Practitioner plays a vital role in care coordination, working closely with specialists, testing facilities, and various agencies to deliver seamless health services. Leadership in population health initiatives is another critical aspect of the position, ensuring patients receive recommended preventive screenings and evidence-based interventions. The provider also supports phone triage, outreach efforts, and may perform home visits as needed to provide comprehensive care.

Reporting to the Center Medical Director, the Nurse Practitioner contributes to the clinic's overarching goal of enhancing primary care quality and population health outcomes. Oak Street Health offers a competitive salary range from $78,331 to $168,714 annually, depending on experience, education, and location. Job Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
  • Active, non-probationary state Nurse Practitioner license
  • Active DEA license
  • US work authorization

Job Qualifications

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
  • National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
  • Active, non-probationary state Nurse Practitioner license
  • Active DEA license
  • US work authorization
  • Experience in primary care such as internal medicine, geriatrics, or family medicine (preferred)
  • Understanding of HCC documentation, ICD-10 coding, and health risk assessments (preferred)
  • Passion for teamwork and cross-functional collaboration (preferred)
  • Desire to be part of an innovative, empirically-guided population health model (preferred)
  • Bilingual proficiency in applicable areas (preferred)

Job Duties

  • Provision of exceptional primary care
  • Conduct office visits for routine and acute issues
