Oak Street Health, a member of the CVS Health family, specializes in providing primary care services tailored to adults on Medicare. At Oak Street Health’s Louisville, KY location, they are seeking a Nurse Practitioner to join their team as an Advanced Practice Provider. This full-time role works Monday through Friday, from 8:00 AM to 5:00 PM, offering a competitive salary range of $240,000 to $270,000, depending on experience and location. This position plays a critical role in the delivery of exceptional value-based healthcare by focusing on keeping patients healthy, happy, and out of the hospital through effective primary care management.
The Nurse Practitioner at Oak Street Health provides comprehensive and equitable primary care independently, while also collaborating with physicians and other care team members when necessary. The role is fundamental in establishing strong, longitudinal patient relationships that build trust and improve patient engagement with their care plans. Nurse Practitioners work within a collaborative outpatient care team model that includes physicians, nurses, medical assistants, medical scribes, and care managers, providing a holistic approach to patient care.
This role has a broad scope encompassing routine office visits, acute issue management, annual wellness evaluations, health risk assessments, and care coordination with specialists, testing facilities, and other healthcare providers. The Nurse Practitioner also contributes to population health initiatives, ensuring patients receive evidence-based screenings and follow-up care. Beyond clinical responsibilities, this position may involve patient education, home visits, and participation in promotional activities that enhance patient outreach and engagement.

Job Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- national certification in at least one approved specialty
- active, non-probationary state nurse practitioner license
- active DEA license
- US work authorization
Job Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- national certification in family nurse practitioner, adult-gerontology primary care nurse practitioner, adult nurse practitioner, or gerontological nurse practitioner specialty
- active, non-probationary state nurse practitioner license
- active DEA license
- US work authorization
- experience in primary care including internal medicine, geriatrics, or family medicine preferred
- understanding of HCC documentation, ICD-10 coding and health risk assessments preferred
- passion for teamwork and cross-functional collaboration
- desire to be part of an innovative population health model
- bilingual proficiency in applicable areas preferred
Job Duties
- Provision of exceptional primary care
- conduct office visits for routine and acute issues
- administer annual wellness visits and health risk assessments with focus on detailed documentation
- coordinate care with other providers, specialists, testing facilities, and agencies
- lead population health initiatives in coordination with the care team
- assist care team with phone triage and outreach
- educate patients on their health conditions, care plans, and treatments
- participate in Oak Street Health promotional activities
- conduct home visits as needed
- perform other duties as assigned
