We are currently seeking an Advanced Practice Clinician (APC) to join our Senior Community Care (SCC) team. This role focuses on delivering care to high-risk health plan members in sub-acute rehabilitation and long-term care environments. The position offers a competitive total cash compensation that ranges between $109,500 to $164,000, which includes base salary and performance-based bonuses, reflecting market conditions, experience, and educational background. As an Advanced Practice Clinician within the SCC program, you will conduct comprehensive patient assessments, including reviews of past medical histories, medication reconciliations, vital signs, and physical examinations. You will employ evidence-based screening tools and conduct point-of-care testing as appropriate. The APC will support patients during short-term rehabilitation episodes and ensure thorough and accurate documentation of clinical findings, including ICD 10 coding. Interdisciplinary collaboration is a key aspect of this role; you will work closely with care teams to coordinate and deliver effective care plans. The role also involves facilitating advanced illness planning conversations and educating patients and caregivers on health management strategies.
