This position focuses on providing comprehensive in-home care to some of the highest-risk health plan beneficiaries throughout Arkansas. The Nurse Practitioner or Physician Assistant will frequently travel statewide, approximately 85% of the time, to conduct thorough assessments and manage both acute and chronic medical conditions in patients' homes. Notably, this role involves no night, weekend, or holiday shifts, supporting a better work-life balance. As part of the Optum Care at Home team, the incumbent will conduct detailed patient evaluations including medical history reviews, medication reconciliations, physical exams, and evidence-based screenings. They will also engage in care planning, manage rehabilitation episodes, administer necessary interventions, and collaborate closely with a multidisciplinary care team to optimize patient outcomes. The role is vital in delivering advanced illness and end-of-life care planning discussions with patients and caregivers, providing education and counseling to support health literacy.

Compensation ranges from $109,500 to $164,000 annually, inclusive of base salary and performance-based bonuses, reflecting factors such as experience, education, and local market trends. In addition to competitive pay, the role offers a generous benefits package alongside options such as a $25,000 student loan repayment or $40,000 sign-on bonus. The employment type is full-time with substantial travel requirements but with weekends off. Job Requirements

  • Active ANCC or AANP certification for Nurse Practitioners
  • Current NCCPA certification and state licensure for Physician Assistants
  • Active unrestricted state licensure or ability to obtain by start date without disciplinary actions
  • Active and unrestricted driver's license with access to reliable transportation
  • Current active DEA licensure or prescriptive authority or ability to obtain post-hire per state regulations
  • Ability to obtain collaborative practice agreement if applicable
  • Ability to navigate varied environments and perform physical duties such as sitting, standing, or kneeling
  • Willingness and ability to travel throughout Arkansas up to 85% of the time
  • No disciplinary actions or practice limitations on licensure

Job Qualifications

  • Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for Nurse Practitioners
  • Current NCCPA certification and state licensure for Physician Assistants
  • Active, unrestricted licensure in the state of practice or ability to obtain by start date without disciplinary actions
  • Current, active DEA licensure or prescriptive authority or ability to obtain post-hire per state regulations unless prohibited
  • Ability to gain collaborative practice agreement if required by state
  • Ability to navigate varied environments and perform duties involving sitting, standing, or kneeling
  • Preferred one or more years of clinical experience as an APC, especially in family, geriatric, or home health care
  • Experience in geriatric medicine, long-term care, senior living or home care settings
  • Understanding of advanced illness and end-of-life care discussions
  • Proficiency with electronic medical records and healthcare technology

Job Duties

  • Conduct comprehensive in-home assessments including past medical history, medication reconciliation, vital signs, and physical exams
  • Perform evidence-based screenings and point-of-care testing as appropriate
  • Manage patients through acute medical events or exacerbations of chronic illnesses including testing and interventions
  • Oversee short-term sub-acute rehabilitation episodes
  • Document clinical findings accurately including ICD-10 coding
  • Collaborate and communicate effectively with the interdisciplinary care team
  • Lead advanced illness and care planning conversations and provide patient and caregiver education
