This role is for an Advanced Practice Clinician within the SCC program, focused on providing care to the most vulnerable senior patients. Candidates who join will experience a supportive work environment offering a $5,000 sign-on bonus for external candidates, with no on-call, weekend, or holiday obligations, creating a balanced professional lifestyle. The position offers a salary range from $53 to $79 per hour, which reflects base pay plus bonuses based on productivity and performance.
The clinician will conduct comprehensive patient assessments, including past medical history reviews, medication reconciliation, vital signs monitoring, and physical exams. Responsibilities also include performing evidence-based screenings, point-of-care testing when appropriate, and supporting patients through short-term sub-acute rehabilitation. In addition to clinical care, accurate and complete documentation—including ICD-10 coding—is essential, along with strong communication and collaboration with interdisciplinary care teams. Clinicians will engage in advanced illness and care planning discussions and provide counseling and education to patients and caregivers to empower them in managing health and wellness.

Job Requirements
- Bachelor’s or Master’s degree in Nursing or Physician Assistant studies
- Relevant national certification as NP or PA
- Active state licensure or ability to obtain by start date
- Valid driver’s license and access to reliable transportation
- DEA licensure or prescriptive authority as required
- Ability to work 24 hours monthly including up to 16 hours during evenings, weekends, or holidays
- Capability to perform job duties across varied clinical environments without limitations
- Strong communication and interdisciplinary collaboration skills
Job Qualifications
- Active ANCC or AANP certification for nurse practitioners in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Current NCCPA certification and state licensure for physician assistants
- Active, unrestricted licensure in the practicing state or ability to obtain by start date without disciplinary actions or practice limitations
- Valid and unrestricted driver’s license with access to reliable transportation
- Current, active DEA licensure or prescriptive authority or ability to obtain post-hire per state regulations unless prohibited
- Ability to obtain collaborative practice agreement if applicable
- Clinical experience as an advanced practice clinician preferred
- Experience in geriatric medicine, long-term care, or senior living environments preferred
- Proficiency with electronic medical records and healthcare technology
Job Duties
- Conduct comprehensive assessments including past medical history review, medication reconciliation, vital signs, and physical exam
