Optum is currently seeking qualified Advanced Practice Clinicians (Nurse Practitioners or Physician Assistants) for its HouseCalls program, a unique, performance-driven initiative that brings annual in-home health assessments directly to members of Medicare Advantage and other health plans. This role emphasizes collaboration among interdisciplinary teams and primary care providers to enhance patient outcomes by addressing healthcare gaps through education and thorough assessments.

This is a field-based, non-prescribing position requiring extensive travel across several states including Alabama, Arkansas, Florida, Georgia, Kentucky, Louisiana, Mississippi, North Carolina, and South Carolina. Travel accounts for approximately 85% of the role, and candidates can reside anywhere within these states. The company supports travel entirely, reimburses all related expenses, and offers a competitive salary plus various performance incentives. Flexible scheduling between 8 am and 8 pm over a seven-day work week allows clinicians to maintain work-life balance with no on-call duties, no weekend requirements, and no holiday shifts.

In this role, clinicians will perform comprehensive in-home assessments that include reviewing past medical history, conducting medication reconciliations, taking vital signs, completing physical exams, and administering evidence-based screenings and point-of-care tests as appropriate. Findings are communicated to primary care providers to coordinate care plans and close health gaps. Clinicians also play a crucial role in identifying urgent medical situations, educating patients on disease management and medication compliance, and addressing social determinants of health by facilitating appropriate referrals and support services.

Compensation for this specialty generally ranges from $109,500 to $164,000, including base pay and bonuses tied to individual performance and local market factors.
