This particular position is a remote, home-based role requiring residency in North Carolina or within 40 miles of the state border. It is a part-time role with a scheduled workweek that includes weekend and holiday hours: Fridays from 5:00 pm to 1:30 am, and Saturdays and Sundays from 8:30 am to 5:00 pm. The pay rate is approximately $25.49 per hour, with eligibility for overtime compensation as the role is non-exempt.
The role of the Care Coordinator under Vaya Health's NCServes Coordination Center is pivotal for providing afterhours support to veterans, service members, and their families throughout North Carolina. This position extends the functionality of the Vaya Health call center by offering timely and accurate assistance outside of regular business hours. The Care Coordinator acts as a vital link between individuals in need and available community resources, ensuring effective referrals and connection to services such as housing, employment, financial aid, social support, and other critical needs. Care Coordinators are responsible for conducting biopsychosocial assessments through telephone consultations to identify client needs across multiple dimensions. The role requires a deep understanding of the network of providers available statewide, including knowledge of service menus, eligibility criteria, and intake procedures. The Care Coordinator must maintain strong relationships with these providers and engage in community meetings to ensure seamless service delivery.
Documentation and compliance with grant program guidelines and internal policies are essential components of the job. The Care Coordinator will regularly document activities, track referrals, and gather feedback from clients and providers to ensure successful engagement and continuous improvement of service pathways. Coordinating with other programs and ministries within ABCCM (The Asheville Buncombe Community Christian Ministry) is also required, fostering a collaborative environment that maximizes the impact of services provided.
This role demands a self-directed and highly motivated individual committed to the missions of both ABCCM and Vaya Health. Excellent communication skills, proficiency with web-based platforms and the Microsoft Office Suite, and prior experience working with at-risk populations are necessary for success. Experience with veterans is preferred but not mandatory. The position reports to the Clinical Director of the MRS (Medical Rehabilitation Services) and interacts regularly with VSC staff, veteran participants, families, community providers, volunteers, and interns.

Job Requirements
- High school diploma/GED
- Minimum of 2 years experience working with at-risk populations
- Must reside in North Carolina or within 40 miles of the NC border

Job Duties
- Conducts biopsychosocial assessments by phone with clients to identify needs in key areas such as housing, employment, financial assistance, benefits, social connection
- Makes referrals to Network providers to address identified needs based on understanding of eligibility criteria, capacity, location, and client preference
- Maintains a working knowledge of Network providers' menu of services, eligibility criteria, intake process and preferences
- Develops positive working relationships with array of Network providers and attends monthly community meetings
- Documents all activity in accordance with grant program guidelines and in compliance with internal policies and procedures
- Elicits feedback from clients and providers to ensure successful engagement
- Coordinates across other programs and ministries within ABCCM
- Participates in all appropriate staff, supervision, and training meetings
- Assumes other duties as assigned by Leadership
