The Community Care Coordinator plays a vital role within the Concierge Care team, reporting directly to the Executive Director. This position is instrumental in ensuring exceptional service delivery to clients and supporting caregivers’ professional growth and performance at one of the company’s premier communities in the western Chicago suburbs. The ideal candidate is proactive, empathetic, and highly organized, with significant experience in senior care or related industries such as private duty or hospitality. The Community Care Coordinator acts as the central liaison between clients, families, caregivers, community partners, and internal teams to ensure smooth communication, tailored care planning, and quality assurance.

Key responsibilities include collaborating with senior care providers like physical therapists, occupational therapists, hospice, and home health professionals to foster comprehensive care. The role involves conducting detailed consultations and assessments with clients and families to develop individualized care plans that address medical, functional, and lifestyle needs. The Community Care Coordinator manages a select caseload of clients, serving as their primary contact and ensuring a seamless, high-quality care experience. They also play an active role in hiring, training, and mentoring caregivers, matching them with clients based on skills, personality, and lifestyle preferences to build strong, lasting relationships.

Quality assurance is a critical aspect of this position, involving regular visits and check-ins to confirm adherence to care plans and client satisfaction. The Community Care Coordinator must anticipate evolving client and family needs, providing proactive solutions and personalized recommendations to maintain excellent service standards. Job Requirements

  • Minimum of 3 years experience in senior care or related client-facing roles
  • LPN certification or equivalent healthcare credential preferred
  • Demonstrated ability to manage complex care plans and client relationships
  • Proficiency in care management software such as WellSky
  • Strong interpersonal and communication skills
  • Ability to work scheduled hours every other weekend
  • Must be able to lift up to 50 pounds
  • Valid driver’s license and reliable transportation required
  • High school diploma or equivalent required

Job Qualifications

  • 3-5 years of experience in a high-touch, client-facing role preferably in senior care, private duty, or hospitality
  • LPN certification strongly preferred
  • Proven experience in care management, managing complex client relationships, and supporting or leading a team of professional caregivers
  • Strong communication, interpersonal, and problem-solving skills with discretion and professionalism
  • Knowledge of ADLs and IADLs for senior care clients
  • Experience working in a private duty or home care service company
  • Exceptional organizational skills and attention to detail
  • Proactive, service-oriented mindset with a passion for providing extraordinary care
  • Proficiency in WellSky or other care management software and Microsoft Office 365
  • Ability to lift up to 50 pounds
  • Valid driver’s license and reliable, professional transportation

Job Duties

  • Serve as the primary liaison with community partners including activities directors, executive directors, resident committees, and sales/marketing teams
  • Collaborate with non-competing senior care providers such as PT/OT, hospice, and home health partners to support comprehensive care
  • Conduct in-depth, personalized consultations and assessments with clients and families to develop tailored care plans addressing medical, functional, and lifestyle needs
  • Guide clients and families through the intake process including agency registration, care planning, and budgeting for ongoing in-home services
  • Manage a select caseload of clients as the main point of contact, overseeing all aspects of their care and ensuring a seamless, high-quality experience
  • Provide coverage for client shifts on an emergency basis when scheduled caregivers are unable to work
  • Support hiring decisions and assist in training and mentoring caregivers to meet high standards of concierge-level service
  • Match clients with caregivers based on skill set, experience, personality, and lifestyle preferences
  • Conduct regular quality assurance visits and check-ins to confirm care plans are being followed and that clients and families are satisfied
  • Anticipate evolving client and family needs and offer proactive solutions and personalized recommendations
  • Act as a dedicated liaison between clients, families, customer service, and scheduling teams to promote clear communication and timely issue resolution
  • Maintain accurate, thorough, and confidential records for clients and caregivers
  • Work scheduled hours every other weekend to support urgent needs and commitment to 24/7 client well-being
  • Partner with leadership to continuously improve service offerings and maintain market leadership in concierge senior care
  • Participate in process improvement, system implementation, and integration projects as a super user of cloud-based business systems
  • Plan, promote, and facilitate at least one event per week at the community level to enhance engagement and visibility
  • Support successful execution of community-planned events and activities as a value-add partner
  • Visit potential clients and families at rehab facilities or hospitals to coordinate safe transitions home and initiate home care services
