The position of Memory Support Coordinator at Brandel Health and Rehab is a vital supervisory role focused on managing specialized programs within the Memory Support area. This role is responsible for the coordination and implementation of therapeutic recreational activities and services specifically designed for residents experiencing cognitive impairments such as Alzheimer’s disease and other dementia-related conditions. Reporting directly to the Administrator - Health Care, the Memory Support Coordinator oversees a team of approximately 8-12 full-time employees, collaborating closely with nursing, life enrichment, social services, and dietary departments to ensure cohesive care delivery.
This role entails direct involvement in residents’ day-to-day care, administration of assessments, and maintenance of detailed records including social services’ documentation and resident progress notes. The coordinator plays an integral part in developing, revising, and enforcing policies and procedures consistent with regulatory requirements and organizational standards. Additionally, the Memory Support Coordinator facilitates training and educational programs on dementia care, ensuring that staff, volunteers, and family members receive appropriate knowledge to support residents effectively.
Key responsibilities include designing and implementing holistic therapeutic activities that foster cognitive, cultural, physical, and psychosocial well-being, while maintaining a resident-centered philosophy. Job Requirements
- Associate's degree or higher
- Experience working with persons with Alzheimer's disease and other dementia
- Supervisory experience in dementia care preferred
- Documented coursework in dementia and ability-centered care
- Ability to understand and demonstrate compassion for dementia clients
- Knowledge of state and federal skilled nursing facility regulations preferred
- Excellent oral and written English communication skills
Job Qualifications
- Associate's degree preferred
- Bachelor’s degree preferred
- Registered nurse with at least one year of experience with Alzheimer's disease and dementia patients
- Minimum of five years experience working with persons with Alzheimer’s disease and other dementia
- At least two years of supervisory or management experience
- Documented coursework in dementia care and ability-centered care
- Excellent English communication skills, oral and written
- Ability to understand and show compassion to dementia clients
- Knowledge of state and federal regulations for skilled nursing facilities preferred
Job Duties
- Coordinate and implement specialized activity-focused programs within the Memory Support area
- Provide direct day-to-day care and activities for residents
- Coordinate training and in-services for residents, family members, and unit personnel
- Develop, implement, and maintain policies and procedures for the program
- Establish and participate in therapeutic recreational activities that promote cognitive, cultural, physical, and psychosocial well-being
- Assess residents with diminished mental function and assure appropriate services
- Act as liaison with residents, families, physicians, staff, and other professionals
- Direct and schedule Life Enrichment programs daily
- Monitor evaluation and effectiveness of programs and revise as necessary
- Maintain unit records including social services documentation and resident progress notes
- Provide orientation, training, coaching, and counseling for unit personnel
- Conduct dementia-related educational programs and in-services
- Ensure communication of resident activity and needs through shift reports
- Manage equipment, supplies, and budget for Life Enrichment expenses
- Act as Volunteer Coordinator and participate in facility quality initiatives
- Assist in emergency evacuations as needed
- Perform other related duties as assigned
