The role of the Medical Social Worker at Oak Street Health is a pivotal position within this integrated care environment. This full-time role involves working closely with interdisciplinary care teams to address the psychosocial needs of patients who are among the highest risk. The Medical Social Worker conducts comprehensive assessments and delivers advocacy and extensive care coordination to empower patients and their caregivers. Key responsibilities include providing in-person and telephonic support, identifying patient goals, addressing healthcare barriers, and coordinating community resources. The position requires expertise in navigating insurance benefits, supporting advance care planning, and educating patients and families on life stage and disease-specific issues such as dementia and caregiver support. The Medical Social Worker serves as a vital connector between patients and the broader healthcare team, ensuring seamless communication and collaboration that drives effective interventions and improved health outcomes. This role reports directly to the Practice Manager and fosters a collaborative approach by partnering with Community Health Workers and other team members. The anticipated weekly hours for this position are 40 hours per week, and employment is full-time. The pay range offered varies from $46,597 to $101,228 annually, reflecting factors such as experience, education, and geography. Additionally, the position is eligible for bonuses, commissions, or other short-term incentives.
