Unlike traditional medical scribes, these Clinical Informatics Specialists not only document detailed patient encounters but also become experts in value-based care models and population health strategies. They assist with chronic condition management, including precise ICD-10 and CPT coding, which is crucial for meeting quality care standards and closing care gaps. Medical Scribes at Oak Street Health undergo extensive training that covers clinical workflows, preventative care practices, population health data, and collaborative team care. Job Requirements
- Knowledge of medical terminology and common medications
- Prior clinical experience preferred

Job Duties
- Documenting patient encounters including history, assessment, plan, and physical exam
- Assigning appropriate CPT and ICD-10 codes
- Preparing after visit summaries
- Requesting and reviewing medical records
- Leveraging population health tools for documentation improvement
- Supporting daily huddles and clinical documentation reviews
- Providing administrative support such as placing orders and addressing tasks
