The role of Medical Scribe, also known as a Clinical Informatics Specialist (CIS) at Oak Street Health, is a vital position that supports primary care providers by managing clinical documentation throughout the patient care journey. This role allows providers to concentrate fully on patient care while the scribe accurately captures all necessary details during patient visits. The Medical Scribe attends morning huddles for visit planning, observes and documents patient encounters in real-time within exam rooms, and follows up with providers to assist with subsequent steps including documentation clarity and clinical coding.
Beyond general scribe duties, Medical Scribes at Oak Street Health take on additional responsibilities such as clinical documentation improvement by utilizing population health data, supporting value-based care documentation, and helping close care gaps. Scribes receive extensive training that covers clinical workflows, preventative care for chronic conditions, coding standards including ICD-10 and CPT, and the nuances of value-based healthcare. This role offers invaluable experience for individuals on pre-med tracks or those pursuing careers in health informatics, public health, healthcare administration, or medical coding.

Job Requirements
- knowledge of medical terminology and common medications from pre-medical degree or clinical experience
- prior clinical experience including shadowing or volunteering preferred
- advanced listening and communication skills
- strong computer literacy
- ability to adapt to new workflows
- ability to take direction and work as part of a care team
- ability to be a self-starter
- excellent attendance and ability to work in-person
- ability to commit to at least 1 year in role
- ability to work full-time hours (40-45 hours per week)
- compliance with HIPAA and other policies
- legal authorization to work in the US
Job Qualifications

- fluency in Spanish, Polish, Russian, or other languages spoken by communities served where required
- ability to adapt to new workflows and learn new concepts

- ability to commit to at least 1 year in role
- ability to work full-time hours during clinic hours
- compliance with hospital and Oak Street Health policies including HIPAA

Job Duties
- documenting patient encounters including history of present illness, assessment, plan, and physical exam
- joining provider in exam room to observe patient visits
- assigning appropriate CPT and ICD-10 codes
- preparing after visit summaries
- consulting with provider to ensure accurate documentation
- requesting and reviewing medical records
- leveraging population health tools to support clinical documentation improvement
- preparing for and supporting daily huddles and clinical documentation reviews
- consulting with provider on documentation opportunities
- placing orders and referrals
- addressing tasks
- supporting care team with responsibilities related to clinical documentation
- performing other duties as assigned
