The Medical Scribe, also known as Clinical Informatics Specialist (CIS), plays a crucial supportive role in Oak Street Health’s primary care teams. Medical Scribes accompany providers during patient encounters, documenting detailed notes such as history of the present illness, physical exams, assessment, and care plans. They assign appropriate ICD-10 and CPT codes to ensure compliance and correct billing. Moreover, they support clinical documentation improvement by utilizing population health data and maintain communication with providers to identify and address care gaps. Beyond documentation, they provide administrative support by placing orders, managing referrals, and helping with various tasks that contribute to the smooth operation of the care team.

Scribes at Oak Street Health receive extensive training on clinical workflows, electronic health records, value-based care, and coding standards, enabling them to become indispensable members of the care team.
