The Medical Scribe, also known as Clinical Informatics Specialist (CIS), plays a crucial supportive role in Oak Street Health’s primary care teams. Medical Scribes accompany providers during patient encounters, documenting detailed notes such as history of the present illness, physical exams, assessment, and care plans. They assign appropriate ICD-10 and CPT codes to ensure compliance and correct billing. Moreover, they support clinical documentation improvement by utilizing population health data and maintain communication with providers to identify and address care gaps. Beyond documentation, they provide administrative support by placing orders, managing referrals, and helping with various tasks that contribute to the smooth operation of the care team.

  • Fluency in Spanish, Polish, Russian, or other community languages preferred

  • Ability to take direction and work as part of a patient care team
  • Ability to be a self-starter

  • Ability to work full-time hours during clinic operations

Job Duties

  • Documenting patient encounters including history, assessment, plan, and physical exam
  • Assigning appropriate CPT and ICD-10 codes
  • Preparing after-visit summaries
  • Consulting with providers for accurate clinical documentation
  • Requesting and reviewing medical records to improve documentation
  • Supporting daily huddles and clinical documentation reviews
  • Placing orders and referrals and addressing tasks for the care team
  • Supporting the care team with additional clinical documentation responsibilities
  • Performing other duties as assigned
