We are currently seeking a Care Facilitator to join our dynamic team. This vital role acts as one of the first points of contact for patients entering the ChenMed family. As a Care Facilitator, you will serve as a patient advocate responsible for delivering exceptional customer service and ensuring smooth operational flow within our medical clinics. Your responsibilities will include greeting patients, managing check-ins and check-outs, scheduling appointments, and supporting various administrative functions. Success in this position requires a strong commitment to superior service, attention to detail, and the ability to connect with patients warmly and professionally.
The role demands proficiency in medical office procedures, familiarity with electronic medical records (EMR), and the capability to handle sensitive patient information with the utmost confidentiality. Among other duties, Care Facilitators coordinate patient schedules to optimize clinical care, verify insurance details, assist with referrals, and contribute to health outcome metrics such as HEDIS reporting. Candidates should possess excellent interpersonal and communication skills, be comfortable using technology including Microsoft Office Suite, and be flexible to work various shifts, including evenings, weekends, or holidays when necessary.
This position offers a competitive hourly wage ranging from $14.30 to $20.42, with final compensation determined by factors such as education, experience, and location. Job Requirements
- High school diploma or equivalent education required
- minimum of 1 year work experience in a medical clinic or similar environment required
- graduation from a nationally accredited medical assistant program preferred
- BLS for Healthcare Providers preferred
- exceptional customer service skills
- working knowledge of medical insurance and/or electronic medical record EMR systems
- proficient in Microsoft Office Suite
- ability to communicate professionally and courteously

Job Qualifications
- High school diploma or equivalent education required

Job Duties
- Interacts appropriately with patients and records information while greeting, checking-in, checking-out or scheduling appointments for patients
- conducts lobby checks to ensure organization and cleanliness
- serves as the gatekeeper for PCP and specialists schedules including ensuring top 40 patients are scheduled every 13 days
- utilizes computer equipment and programs to add new patient information, prints out relevant forms for processing, schedules appointments, responds to emails and generates new patient packets
- ensures patient contact and emergency contact information is accurately documented in the system
- conducts "love" calls, mails birthday and greeting cards to patients commemorating special events
- answers telephones, takes detailed and accurate messages, and ensures messages are directed to the appropriate individual
- clearly communicates information about ChenMed clinical personnel to patients and other individuals
- verifies patient insurance via phone or website and collects necessary copays for services
- assists with scheduling follow-up and in-house specialist referral appointments when necessary
- escorts patients to exam rooms and ensures their comfort and well-being prior to leaving
- acts as HEDIS champion for the PCP by providing visibility of HEDIS needs for scheduled patients
- reports on hospitalizations including HITS census and ENS Ping
- presents patients with customer service survey during check out and escalates if needed for immediate service recovery
- participates in daily and weekly huddles to provide details on patients
- regulates and monitors aspects of transportation including drivers lists, medication deliveries, grocery deliveries, and scheduling Uber/Lyft rides for patients
- maintains the confidentiality of patients' personal information and medical records
- reviews patients' personal and appointment information for completeness and accuracy
- transmits correspondence by mail, email or fax
- performs other duties as assigned and modified at manager's discretion
