must be 21 years of age or older
must have a valid driver's license
must have own minivan
must pass background check and motor vehicle record check
willingness to obtain TSA and customer specific medical certifications
proof of insurance with 100/300 liability coverage
workers' compensation or occupational accident insurance as per state statute or $1MM occurrence/accident coverage
