Job Description

Community Medical Centers (CMC) is a leading healthcare provider dedicated to improving health and well-being in the communities of San Joaquin and Solano counties. The position available is for a Medical Assistant (MA), a crucial member of CMC's Care Team. The MA role involves working closely with licensed clinical staff and other healthcare professionals to deliver exceptional medical care and an excellent patient experience. Key responsibilities include conducting thorough intake assessments, managing pre-visit chart preparations, administering vital signs and screenings, facilitating preventive care, and providing direct assistance during patient visits. The role supports a multi-disciplinary team approach, ensuring patients receive coordinated and timely care. Reporting to the Center Manager or Back Office Supervisor and under the clinical oversight of the Chief Nursing Officer, the MA is essential to maintaining efficient workflows and supporting CMC's quality standards.

This full-time position encompasses diverse duties such as setting up Telehealth appointments, performing laboratory testing including CLIA-waived tests, administering immunizations, managing electronic health records, assisting with patient education, and supporting referral processes. The role requires flexibility, as the MA may be scheduled to work at different clinic locations and may contribute to front-office functions when needed. The ideal candidate for this role will have completed an accredited medical assistant training program, hold a high school diploma or equivalent, and either possess or be able to obtain current basic life support certification. Physical demands include the ability to perform a wide range of motions including lifting, standing for long periods, and working with various medical instruments and equipment. Job Duties

  • Promotes the mission, vision, and values of the organization
  • conducts intake assessments including reason for visit, histories, medication reconciliation, vital signs, and consents
  • sets up Telehealth appointments and performs intake assessment remotely
  • conducts pre-visit chart scrubbing to identify follow-up, screenings, or preventive care needed
  • ensures completion of screening tools such as PHQ 9 and Health Literacy
  • facilitates team adherence to "Team Care" best practices with daily and ad hoc huddles
  • provides red carpeting services to patients
  • identifies and documents patient self-management and follow-up goals
  • prepares examination areas and assists with immunizations and procedures
  • assists in maintaining patient flow and communicates delays
  • provides interpretation services when needed
  • administers injections and collects specimens as ordered
  • performs and documents CLIA-waived laboratory tests
  • enters records into Electronic Health Records for encounters
  • facilitates communication between providers and patients
  • works independently and takes direction from clinicians
  • maintains supplies, sterilizes instruments, and monitors equipment function
  • coordinates referrals and follow-up care
  • delivers patient education
  • performs clinic opening and closing procedures
  • travels to different sites as needed
  • participates in staff meetings and communication activities
  • ensures clinics meet quality metrics
  • completes reportable disease records and monthly reports
  • supports front-office functions when required
  • complies with all other duties as assigned
