The LTSS Service Coordinator - RN Clinician role is a key field-based position within Elevance Health, primarily focused on providing comprehensive case management services to members residing in specific counties in Texas, including Lubbock, Hale, Wilbarger, Potter, Randall, and Wichita counties. This role involves traveling up to an hour daily to client sites or designated locations, ensuring direct engagement and responsiveness to member needs. The position supports a hybrid workforce strategy, requiring occasional office attendance for meetings or training but largely operating in the field for a more hands-on approach. Candidates are expected to live within a reasonable commuting distance or obtain accommodations as required by law.

The RN Clinician serves as the central clinical authority for managing members' cases within their licensure scope, overseeing the work of non-RN clinicians and coordinating care plans that holistically address members' physical, behavioral, and social service needs, including long-term services and supports. This role requires conducting detailed face-to-face clinical assessments, developing individualized care plans with measurable short and long-term goals, and adapting these plans in collaboration with the member and care team. The focus is on optimizing health outcomes and ensuring cost-effective utilization of benefits while identifying members at high risk for complications to intervene appropriately.

The RN Clinician also navigates complex healthcare ecosystems by facilitating authorizations and referrals to necessary services within or outside contracted benefits and collaborating closely with medical directors, physician advisors, and interdisciplinary teams. This role includes problem-solving service delivery or claims issues and supervising licensed professionals such as LPNs, social workers, and others, guiding their contributions to member care and monitoring their performance. A flexible, collaborative mindset and advanced clinical judgment are essential to succeed in this dynamic role.

Job Requirements

  • Current, unrestricted RN license in applicable state(s)
  • Minimum of 3 years experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in service coordinator or case management roles
  • Resides in or near Lubbock, Hale, Wilbarger, Potter, Randall, or Wichita counties in Texas
  • Ability to travel up to 1 hour daily
  • May require state-specified certification based on state law or contract
  • Willingness to work primarily in the field with occasional office attendance
  • Compliance with vaccination requirements as applicable
  • Ability to communicate effectively with members and care teams
  • Strong organizational skills

Job Qualifications

  • Registered nurse (RN) license in good standing
  • Minimum of 3 years experience in case management, service coordination, or a related role working with individuals with chronic illnesses, co-morbidities, and/or disabilities
  • Knowledge of care plan development and clinical assessments
  • Ability to travel up to 1 hour daily within designated Texas counties
  • Proficiency in coordinating multidisciplinary care teams
  • Strong communication and interpersonal skills
  • Preferred master’s degree in health or nursing
  • Familiarity with applicable state laws and regulations governing service coordination

Job Duties

  • Identifies members for high risk complications and coordinates care in conjunction with the member and the health care team
  • Manages members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost effective and efficient utilization of health benefits
  • Obtains a thorough and accurate member history to develop an individual care plan
  • Establishes short and long term goals in collaboration with the member, caregivers, family, natural supports, and physicians
  • Develops the care plan for services and ensures member access to those services
  • Facilitates authorizations/referrals for utilization of services within benefits structure or through extra-contractual arrangements
  • Interfaces with Medical Directors, Physician Advisors and/or Inter-Disciplinary Teams on care management treatment plans
  • Provides supervision and direction to non-RN clinicians working on member cases
  • Assists in problem solving with providers, claims or service issues
  • Reviews and evaluates non-RN clinicians’ performance regularly
